We are frequently asked what is the difference between Dragon Medical One and Dragon Medical Practice Edition 4. Let’s start with Dragon Medical Practice Edition 4 as it was released in January 2018 and this is the first update to the software in about five years.

Dragon Medical Practice Edition 4

Dragon Medical Practice is based off of the same technology used for Dragon Medical One, so they both offer similar performance. The main difference between the two products is this: Dragon Medical Practice is installed and runs locally on your computer’s hard drive. It uses your computer’s local resources for memory, processing power, and storage. Your voice profile will only be accessible on that particular workstation (the one Dragon Medical Practice is installed on). On another note can install the software up to four times. So if you have multiple exam rooms, would like to do dictation from home, or want to install it on a laptop while you’re traveling, you can do these things. The recognition is quite phenomenal, within 30 seconds can set up your user ID and be up and dictating. Nuance has spent a lot of time in research and development to get to this point. Many years ago you had to read different sentences for the software to learn your voice patterns. This is no longer the case. It accounts for accented users, non-native English-speaking users, as well as other dialects.

Dragon Medical One

As of this writing, Dragon Medical One has been available to the market for just over a year. It offers some distinct advantages over Dragon Medical Practice. The biggest appeal is that your profile is stored in the cloud, meaning any computer you have access to can use your medical voice recognition profile. All of your customized words, and templates (word expansions) are available anywhere. Additionally, your voice profile is automatically backed up and protected by being stored in the cloud. The pricing for this model is different, as you pay a monthly subscription fee. But paying the fee has its advantages. With the monthly subscription fee, the automatic backup of your voice profile, as well as access to all the latest and greatest updates for improvements to speed accuracy and language modeling, are included.

Voice Recognition for Mobile Devices

Most software that caters to computers will not work for your mobile phone. For that, you will need to download a separate app which will take care of your dictation needs. For quick voice to text needs, these will come in handy, but it is best to use the computer version when you have a lot of work. If you are looking for a mobile dictation app, your best options are:

  • Google Now

Working in the medical field, you should know that these apps are not HIPAA compliant (read: Summary of the HIPAA Security Rule), so it may be illegal to use them for medical purposes. Dragon Medical User Profile Errors can be a real pain:

Overview

The following errors can be presented when a user attempts to open a Dragon Medical User Profile:
The user files you have selected cannot be accessed. This could be because you do not have sufficient privileges to use them or because they are currently being accessed by another program.

The local cache files for the user you have selected cannot be accessed. This could be because you do not have sufficient privileges to use them or because they are currently being accessed by another program.

User <name> is locked, and thus cannot be accessed
Your profile is locked. Do you wish to continue?
There can be multiple causes for the above errors.

Solution

Refer to the recommendations listed below in order to eliminate common causes for these profile access errors:
1. Close the same Dragon user logged in at any other workstation(s).
2. Confirm that the Profile folder structure is not being accessed by another application, including Windows Explorer or any text editor. Windows Explorer from any workstation accessing the folder structure can lock the Profile and therefore it should be closed.
3. Check the profile folder structure for any username.lck files, or folder/file names with _DgnRenamed appended.

username.lck
The username.lck file is commonly found under the Profile’s \current sub-folder and can simply be deleted. Search for this string over the entire profile folder structure.

_DgnRenamed
This character string can be appended to the name of a profile sub-folder or file.
It is commonly seen as \[input device]_container\aoptions.ini_DgnRenamed or \current\options.ini.DgnRenamed.
Search for this string over the entire profile folder structure.
 If the unappended file is also present, compare the modified dates for both files and retain the most recent file which will require renaming both files.
 Otherwise, if the unappended file is not also present, simply rename that file by removing the “_DgnRenamed” string.
 If a folder name is appended, simply rename that folder by removing the “_DgnRenamed” string.
Dragon places a lock on the folder structure and certain files while synchronizing, saving, and updating the Profile. The presence of the above appended files/folders means that process was somehow interrupted. The causes include:
 The user shutting the workstation down ungracefully, resulting in Dragon not being able to complete its own proper closure of the user’s profile (most common)
 Network drop-out / latency issues

4. Verify that each Windows user account has the appropriate Read/Write/Modify NTFS permissions to the entire Dragon local/locally-cached, and master roaming folder structure (including the Delete Subfolders and Files permission).
5. Add the proper file exclusions for Dragon Medical to all antivirus/malware/security solftware. This should include the complete profile folder structure for all sub-folders and files:

C:\Program Files\Nuance
C:\Program Files\Common Files\Nuance
C:\Documents and Settings\All Users\Application Data\Nuance
C:\Documents and Settings\\Application Data\Nuance

Windows Vista / 7 / 8

Customer’s local cache location (if configured for a custom value and different from the above defaults).

Customer’s master roaming profile location.
6. Temporarily disable Antivirus Realtime or Endpoint protection modes to isolate this as a cause.
7. Eliminate any other application or process from locking that profile folder structure (including Windows Explorer, Dragon Acoustic and Language Model Optimization, Antivirus/Security/Malware software, IT backup software, etc.)
8. Try renaming the locally-cached Dragon profile on the workstation. DMEE (Enterprise Edition) Roaming configurations and DMNE (Network Edition) maintain a copy of the master roaming profile on the client workstation.
9. Review the Windows Event Viewer logs on the client workstation, and if applicable on any servers where Dragon profiles are stored, for evidence of errors or interference with Dragon Medical by another process.
If the problem’s source cannot be identified, then the next time this occurs, Voice Automated Technical Support will need to review the dragon.log file and remote into the workstation to look at the profile folder structure.

Dragon Medical Practice Edition – iPhone and Smart Phone Apps Free Doctors to Dictate Anywhere

At Voice Automated, we had a focus group a few years back on creating the ultimate medical dictation and electronic record application.

After some time brain storming on the white board, we came up with a light weight device the floats around the doctor’s head and when they talk, the device instantly translates their speech into usable patient information. Oh yea and I forgot, it’s free!

Well we are not there yet but with the latest announcement about an advanced suite of Dragon Medical Mobile technologies and solutions from Nuance we are one step closer to our white board dream detailed above.

I had a chance to view this technology on a recent trip to Boston and it looks promising.

To get an idea of how the technology will work, be sure to try the General English version of Dragon Dictation (available for the iPhone) to dictate short emails or text messages. A few friends of mine, that know I sell Dragon Medical Solutions, asked me about that application at a restaurant. I downloaded the application onto a friend’s iPhone in a busy restaurant and dictated an email at 98% accuracy. I then handed the iPhone back to my friend and he dictated another email with the same result. Absolutely amazing!
