Dragon Copilot 4.0 is the first release to cross into a new major version — and it earns the number by changing what a finished note looks like. Earlier releases focused on capturing the encounter; 4.0 focuses on structuring it: standalone care plans, template-formatted plan sections, diagnosis-level specificity, and a time-ordered patient history. Nursing, meanwhile, reaches general availability and gains controls of its own.
New for physicians
Structured care plans, standalone
Generate structured treatment and care plans directly in Dragon Copilot — no EHR integration dependency. Note section templates now format the plan section to your specialty or style.
Diagnosis specificity
Evidence-grounded, diagnosis-specific suggestions help capture precise detail at the point of care — which Microsoft positions as supporting compliant coding accuracy. It ships off by default, so you evaluate and train before enabling.
A chronological patient history
A new chronological HPI style writes the History of Present Illness as a time-ordered narrative, from onset to the present visit — especially useful for complex or longitudinal cases.
Steadier multi-step workflows
Dragon Copilot’s multi-step workflows now run more reliably inside the EHR, without inserting manual pause steps — less manual intervention, fewer misfires. Requires the latest desktop client.
Voice in web Outlook & Word
The desktop client now brings voice-driven dictation and commands to the browser-based versions of Microsoft 365 Outlook and Word — the same way it already works in the desktop apps.
New for nursing
Full flowsheet visibility (GA)
Nursing documentation now shows all available flowsheet rows — not only the ones with prior documentation — so admission and administrative charting is faster and less is missed.
Clear notice when LDA is off (GA)
When Lines, Drains, and Airways capture is disabled, nurses now see an on-screen notification on the recording screen — real-time transparency into what is and isn’t being documented.
Now in preview
Three capabilities enter public preview: a configurable shift-inactivity timeout, two ready-made library prompts (summarize patient education / latest interaction), and a hide-transcripts control.
For administrators
Centralized management view
One place to onboard organizations, manage user accounts, and configure system-wide settings — less setup complexity, more visibility and control.
Dedicated nursing access role
New role-based access lets you assign nursing users to specific environments, individually or via security groups. Note the 60-day window to complete assignments.
Self-service nursing controls
Manage LDA functionality by environment or group, and set the shift-inactivity timeout yourself — adjustable from 8 to 16 hours, defaulting to 10.
Two-dashboard analytics
An enhanced experience: the familiar Overview for adoption and license usage, plus a new User engagement details view — with date-range and group-by controls.
Clearer settings, fewer toggles
Setting labels are refreshed for clarity (for example, Customizable templates is now Note section templates). On the nursing side, the generative-AI toggle is gone — it’s simply always on.
Support for extended-EHR affiliates
Licensing and management support for arrangements where a host health system extends its EHR instance to partner or affiliate organizations, including shared, multi-tenant environments.
