Dragon Copilot’s clinical notes change on September 30A context-aware HPI, a rebuilt psychology note, and a leaner psychiatry HPI — applied automatically, with no version number attached

Three note-output updates begin reaching US physician and clinician customers on Wednesday, September 30. Nothing to install. Something worth checking.

Talk to us about what changes →

The short version: this is not Dragon Copilot 5.2, and there is nothing to install. Microsoft applies these changes to the AI models automatically, starting September 30 in the US. If you document with Dragon Copilot — and especially if you practice psychology or psychiatry — your notes will look different afterwards.

Where this sits

Dragon Copilot has numbered releases, and it also has model updates that arrive between them. This is the second kind.
Current release
5.1
Hands-free “Hey Dragon” recording, foot pedal support, and AI template workflows.

Context-aware HPI, a rebuilt psychology note, and a simplified psychiatry HPI. Automatic, US, from September 30.

Next
5.2 — planned
Planned for October: patient matching, usage analytics, and Android and Spanish for nursing.
The pattern is worth knowing: in 2026 some of the most noticeable changes to Dragon Copilot arrive between numbered releases, applied to the model rather than shipped to your desktop. The August psychiatry update was one. This is another.

What changes on September 30

Three updates to what Dragon Copilot writes, for physician and clinician customers in the US. Microsoft applies them automatically and describes them as requiring no action.
🩺

A context-aware HPI

Where your EHR integration has diagnosis retrieval enabled, Dragon Copilot now uses the visit’s diagnoses to tailor the History of Present Illness.

🧠

A rebuilt psychology note

More emphasis on therapy content throughout the note, a stronger Mental Status Exam, and a more comprehensive Assessment & Plan.

📝

A simplified psychiatry HPI

Three HPI sections are removed as separate sections, and Family History is added.

The HPI now reads the visit’s diagnoses

This is the change most clinicians will notice, and it applies across specialties — but only where one integration setting is in place.

When diagnosis retrieval is enabled in your EHR integration, Dragon Copilot takes the diagnoses attached to the visit into account when it writes the HPI. Microsoft describes the result as better capture of the relevant clinical details, medications, and dates for those diagnoses — while summaries of conditions unrelated to the visit are maintained.

In practice, that means an HPI organized around why the patient is actually being seen, instead of an even-handed retelling of everything that came up in the conversation.

Diagnosis retrieval is not available in every EHR integration, and where it is available it has to be turned on. If you are not sure whether your organization has it, ask us — it is a quick check, and it decides whether this update reaches your notes at all.

The psychology note, section by section

If you document psychology encounters, this is the largest change in the update.
💬

Therapy content throughout

The note places greater emphasis on therapy content throughout.

🔍

A stronger Physical Exam and MSE

The Physical Exam section is improved, with enhanced Mental Status Exam documentation.

📋

A more complete Assessment & Plan

The A&P now captures the problems addressed, therapy content, therapeutic interventions, treatment plans, additional clinical notes, risk factors, and time documentation.

Time and risk factors are the two worth singling out: both are now named parts of what the Assessment & Plan captures, rather than depending on how the conversation happened to go.

The psychiatry HPI, simplified again

If you read our August coverage, this one needs context: it adjusts the psychiatry note model that changed on August 26.

In August, the psychiatry HPI expanded to take in functional status, triggers and stressors, and a wider sweep of history. This update simplifies it. Functional Status, Triggers and Stressors, and Family Psychiatric History are removed as separate HPI sections, and Family History is added.

Microsoft’s notice describes which sections change. It does not say whether the content of the removed sections now appears elsewhere in the note, so this is the part to check against your own first notes rather than assume.

Need the August background?

The August 26 update rebuilt the psychiatry note model and standardized diagnosis naming to ICD-10.

What to check in the first week

There is nothing to install and nothing to approve. There is something to verify.
✅

Confirm diagnosis retrieval

Find out whether your EHR integration has diagnosis retrieval enabled. If it does not, the HPI change will not reach your notes.

📖

Read your first notes closely

Compare a few HPIs, and a few psychology or psychiatry notes, against what you would have documented. Structure changes are obvious; omissions are not.

🗣️

Tell your behavioral health team

A note that changes shape without warning reads as a malfunction. A short heads-up to psychology and psychiatry prevents a support ticket.

⚙️

Check templates built on the old structure

If templates or downstream processes rely on the previous psychiatry HPI sections, check them against the new ones.

Where this fits in your move from Dragon Medical One

Frequently asked questions

What is changing in Dragon Copilot notes on September 30, 2026?

Three automatic note-output updates for physician and clinician customers in the US. First, a context-aware HPI: where diagnosis retrieval is enabled in the EHR integration, Dragon Copilot uses the visit’s diagnoses to tailor the History of Present Illness. Second, a rebuilt psychology note with more emphasis on therapy content, an improved Physical Exam section with enhanced Mental Status Exam documentation, and a more comprehensive Assessment & Plan. Third, a simplified psychiatry HPI.

Is this Dragon Copilot 5.2?

No. These are note-output updates applied to the AI models between numbered releases. Dragon Copilot 5.1 is the current release, and 5.2 is planned for October.

Do I need to do anything to get these updates?

No. Microsoft applies them automatically and describes them as requiring no action. The one dependency is the context-aware HPI, which only applies where diagnosis retrieval is enabled in your EHR integration. What is worth doing is verification: read your first notes after the update against what you would have documented yourself.

What does the context-aware HPI actually do?

It uses the diagnoses attached to the visit to tailor the HPI, improving the capture of relevant clinical details, medications, and dates for those diagnoses, while maintaining summaries of conditions unrelated to them.

What does the psychology note’s Assessment & Plan now include?

Problems addressed, therapy content, therapeutic interventions, treatment plans, additional clinical notes, risk factors, and time documentation.

What changed in the psychiatry HPI?

Functional Status, Triggers and Stressors, and Family Psychiatric History are removed as separate HPI sections, and Family History is added. This adjusts the psychiatry note model that changed on August 26, 2026.

How do we stay ahead of changes like this?

Track them through a partner who does. Voice Automated maintains a running record of Dragon Copilot changes — numbered releases and the model updates that arrive between them — and tells the practices we support what actually changed and what to check.
